4Runner Wolff

The past several weeks i have been doing all i can to try and find the reason why my 4Runner has been throwing trouble code 25. From what I've gathered code 25 means the engine is running lean, and the most common cause of this is a vacuum leak.

Along with this code the truck has been running a little off. when starting it up it idles between 1250 and 1500 rpms. I'll sit and let it warm up but it rarely drops below 1250.when i put the car into drive or reverse it slams into gear, i'm assuming this is due to the high rpms. when i begin to drive the rpms continue to rise sometimes above 4000 before it slams into second gear. from there it shifts rather rough.

I have looked at every vacuum line i can find and that is mentioned in every diagram and cannot seem to see anything wrong with any of them. I basically drown under the hood in carb cleaner but the idle didn't change at all. I'm basically in need of any suggestions as to what could be causing this behavior and throwing code 25.
