1991 22RE won't run right.

This is my second post here. I just joined. Great site. My newly aquired 1991 5 speed standard 22RE extended cab Toyota pickup starts right away, every time, but sometimes the RPM's will surge from a stable level to a higher fixed level, & do it over & over. Other times it's ok. I have to sometimes put it right to the floor, just to get power. It takes a lot to keep it from stumling. No black smoke, just like not taking fuel. Had a tune up done: New wires, plugs, rotor button, & distributor. It can happen anytime, right from cold, through the full warming up temperature range. Unsure if, or when timing chain was done, & apparently, it has a bad rod bearing, (see my first post helping a guy here). Piston is tapping the head. Not sure if the timing is wandering due to these other issues. Also the idle adjust on the intake plenum has been fooled with. How do I adjust this? How can I get the truck to run right? I'm going to get an engine, after it blows up. Thanks for looking. Dani.
