Jumpy acceleration

Jumpy acceleration
Ok here's the situation... I have a '95 4Runner, 6cyl, 3.0 liter, 5 speed, manual trans, 108k. When accelerating from a stop the vehicle hesitates and jerks slightly before smoothing out at a little higher speed, when I shift into 2nd gear and start accelerating again it goes through this little jumpy acceleration process again, this occurs in all gears when acclerating. I really don't have the slightest idea what could be causing so any ideas would be great...if I had to make a SWAG I'd say it feels like the clutch is slipping slightly or something. The closest feeling I could describe it as is when you have small engine and you revv it up with the mixture a little rich and it gets kinda bumpy before smoothing out (I know that's a pretty distant comparison but I figured it might convey the mental image, lol)... sorry for the vague description but it's the best I can do any ideas would be great

I recently had a similar problem with my 98 4runner. I took it to the dealer and there turned out to be problems with one of my cylinders, wires were rusting and fraying, throttle body and also fuel filter. I would suggest having a dealer or shop check it out.
