Hard start and ticking lifters

Hi Toyota crew, I have a 93 4Runner v6 5 speed. 145k on everything. When starting, it will idle at 200-400rpm for 2-3 seconds and then go up to about 1050rpm this is the norm unless the truck has been driven within the last hour or so. In that case it will pop up to 1050rpm right away. Another issue, let’s say I'm driving and take the truck out of gear and coast, the rpm will fall to 1300-1500 and stay there, If I rev up the motor (even very slightly) and quickly take my foot off the gas the idle will usually go down to about 900-1100. It will sometimes fall from 1400 to 900 after a few seconds of coasting without having to rev up the engine. This is with the idle screw almost completely closed shut, strange to me???. I have been driving to truck 8-20 miles a day. Two days ago I started it up and the engine was shaking and seemed to be running like crap just sitting there with idle at 1000 or so. It was doing the same thing when I would rev it up a little or accelerates the truck. Shaking could be felt in the truck and on the gearshift. There is a ticking sound which I think is a lifter. I have not been driving is because of this. It is taking 2-3 second cranking the starter to get the truck to start now. The truck has clean oil, air filter and new spark plugs. I have inspected what I can, I do not see missing or unplugged vacuum lines on the intake manifold. It any one has suggestions or thinks they could help me and this 4runner please post up.
