I'm not too familiar with automatics, but I beleive it may be your governor. The governor is basically in charge of what shifts are to be allowed based on your selection. If the tranny is in "1", it does not shift. If the selector is in "2" it will begin in first gear, shift to second, and not shift at all. Etc... It controls the shifts.
Drag racers purposely disable the governor on automatic transmissions. Correct me if I'm wrong, but I think governors hinder how fast the tranny can spin inside or somethign of that caliber so that's why they remove it.
But yeah, I say the governor if it actually starts out in 3rd instead of 1st.

Shifting
A340 automatic transmissions are shifted by a computer. There are two shift solenoids and one lockup solenoid. I would guess you have something wrong with a solenoid or wiring. The solenoids can go bad, there are aftermarket vendors who sell them a lot cheaper than Factory Toyota. Do a search and you should be able to geet more info.

When I bought my 86 it wouldn't shift on its own , Took it to a tranny shop and the guy told me to go have the truck tuned up.Computer tranny. I did what they said when I got it back from the tune up place tranny worked fine. This my or may not be your problem ,just thought I would tell you what happened with mine

Before you spend the money on changing the selenoids, first try unplugging all the electrical connections going to the transmission and transmission computer, cleaning them, and reconnecting them. I spent the cash on selenoids for my daughters 86 4Runner, and it didn't fix a thing.. took it to a transmission shop and they just unplugged all the connections cleaned them and reconnected them..and it working just fine. I would try that first.
