Perhaps your fuel injector was unplugged cuz when you take off the throttle body to get at the coolant hoses u need to unplug the #1 fuel injector (i had to on my truck when taking the tb off to clean it)

and wuts this seafoam??

Chapman88SR5 you sloved the problem before I could tell you my experence. I had the exact same issue as you and the exact same fix.

Just throwing this out there... My 94 22RE 4x4 has a 17 gallon tank. The gas gauge goes WAY lower than the E. With the needle completely on the left side of the E, I can usually still put in 15 gal. Haven't run outta gas yet.

And it sounds like the #1 injector was unplugged...

Never assume anything. most places that install tires leave them at 40+psi.... for EVERY car.

And my guess would be that you have a bad fuel injector. or multiples. have you have them checked or tested them to be in good working order?

One other thing to check in these absurdly low mileage issues is the fuel pressure regulator. If the diaphragm inside has deteriorated, you may be sucking raw fuel through the vacuum line... Just a thought.
