Truck hasn't got much use but since replacing the head gasket I haven't had any coolant consumption, surging, or lack of heat... it still has a high idle but I think that's due to the IACV.
I don't know about the "no heat" symptom but I'm wondering if the gauge goes up on the highway? Does it want to shift out of OD and into 3rd? IF that is the case then a caked-up radiator is likely. I know you said it was tested for "blockage" but I'm not sure how you can test for crud that is insulating the heat from escaping.

Heres my thread if you have some suggestions. Radiator, 3tStats, flush heater core, engine, and both radiators each separate, 2 block tests, 2 coolant system pressure tests, compression check, much burping, a huge headache, and more described over there.

water pump or head gasket or block coolant passages clogged, or other?

are you talking to me or someone else? because I solved my issue over a year ago, it was a headgasket.
