Leaking extention housing seal

My extension housing seal was leaking and there was a lot of play in the shaft coming out of the housing. So I pulled it apart and got a new bushing pressed in and new seal and its leaking worse then before.

I wasn't sure where this felt pad and metal deal goes do I need to replace it? I couldn't find it on an exploded view or parts store?

My truck is 1988 with auto trans 2wd. Where do you guys get your seals from? any ideas on what im doing wrong?

The seal I bought fits perfect into the housing but the shaft isnt snug in the seal..

who pressed the new bushing in? its hard to tell from the picture, but the yoke looks like it has scoring from riding on the seal. try taking some emory cloth and smoothing that yoke out. i also lightly coat the yoke and the inside of the lip of the seal with some clean tranny fluid before i install the driveshaft to prevent the seal from ripping or rolling.
