95 Toyota 4 Runner Trouble Shooting

I've had this for about 2 weeks. I wanted other opinions on what might be wrong with this. What are some of the meant things that have gone wrong at 200k? I've noticed it pops out of first and second sometimes; I was told by one guy that there's a bearing in the stick shift area. It runs a little rough thru 1st and second. Over all not a lot of get up and go (Power). I was told they have problems around 200k with valves? What mileage are you typically getting. Around town I'm getting 12mi. What does a typically tune-up cost and does it the 100k plugs I read about somewhere. Is that an easy task for a novice?
thanks for any feed back.

Hi,

What motor do you have?

So, you are at 200k and have a 5-speed manual? FWIT, I have lost two Toyota 5-speeds at about 200k ('83 and '87).
