Ok once the truck warms up does it drive fine? Do you let your truck warm up at all when you first start it up? Where do you live (how cold is it in the mornings)? Let's start there.

And welcome to YotaTech!

My 99 4runner is doing this exact thing. I let it warm up for a good couple of minutes and reverse acts fine out of the driveway. When I put it in D, it barely goes for about 20 - 30 seconds, then slowly starts gaining speed and then its fine. Doesnt feel like its out of gear, it doesnt suddenly jolt forward, just real slow going, but the RPMs increase with more gas, but no go.

Any suggestions ???

Yeah, that has been waaaaaay over due. Check your transmission fluid. Hopefully, the damage isn't permenant. Do a flush.
