Arghh, need simple Word template for 4x4 runs

Arghh, need simple Word template for 4x4 runs
I have a list now I had printed out that I write down each rig owners name, passengers, make of rig, ect, but it is in landscape mode, and I would prefer in in the taller portrait format.
I suck at Office Word, I even looked up online at tons of Microsofts templates that people submit.
All I am looking for is a place at top so I can insert a simple logo, then below that one line that says Location and date of run, then below that I need two columns.
One column to insert the names of the party, the next column the type of rig.
I can make columns, but it will not let me insert text into the next column, it wants to keep doing it on the left side of the screen.
I also want vertical lines going down the paper so it separates the column.
This is kind of what I am looking for.

The logo will be different than that one, and I can see the vertical lines I made above do not line up right in this format.
Is there a downloadable template that will do this, or an easy way in Word?
