What causes spark plugs to be blown out?

A friend of mine has a 1998 Tacoma with the 3RZ, 5speed, 4WD. Tonight, I came up on him sitting on the side of the road, and his truck had blown the second plug out. Being that it was already stripped out, we put it back in and I followed him home. Just wondering, what would cause it to blow the plugs out? I know he can put a helicoil in there, but if there is a problem, eventually it is going to do it again.

I know it will need a helicoil and the threads are definitely stripped, but he claims that it wasn't stripped before this. Is there something that can cause it to blow it out with decent threads, or was it probably already stripped and he just didn't know it?
