Re-gearing and vehicle weight question

I searched for this answer and even posted in am old thread someone else created. I got no response so I am starting my own thread.

Anyway I have 33" mt's and I know thats 4.88's are the recommended gearing. What I want to know is does that take in to account the extra weight of bumpers, sliders, skid plates, winches, trail gear a etc?

Also most of the time when I drive my truck I have 2- 5 people in it with lots of gear or bikes. So I was thinking 5.29's might be a better option.
I don't do very much highway driving except Mt Baker HWY and most of the terrain is hilly or mountainous.

Gear ratio choices don't take into account weight. The RPM will be about 200 higher, but will move the truck quicker, but also be higher when running the highways.
