Hey guys, first time poster here. I love this site and all the great info and sage advice going around.
I have a 92 4runner sr5 v6. I love my truck and it is my first vehicle I have done almost all the work on myself and I'm trying to keep it that way.
This issue might seem really easy to some and I would try to test out everything myself, but I don't have the money to check all the options after my blown head gasket that just happened.
So the issue is with a lack of fuel getting into the engine at startup. When I try to start her up in the morning it takes about 30-45 secs with my foot on the gas and the starter going to get it up and running. Once the engine is rumbling a little bit I have to keep the throttle down while it slowly gets more fuel. It takes another 15 secs for it to get to "running" rpm which for some reason has jumped sky high (1750) since the gasket replacement.
Then when I try to get moving the first couple throttle presses in gear drops my rpms to almost nothing. After acouple seconds that clears up and it runs fine. When I try to start her again after this she starts up no problem. Only after the engine has sat for a couple hours does it cause troubles, but mornings are the worst.
I was planning on changing the fuel filter again which I have done already acouple months ago. Then was possibly going to check the fuel pump too, but I'm thinking there might be a fuel injector issue now.
I'm sorry bout the novel here, but I like being thorough. Hope someone has some insight so I can get my girl up and running clean again. Thanks.

if it's an efi truck, don't push on the gas pedal when first trying to start it... pumping the pedal is for carbed vehicles, it squirts gas into the carb, which won't happen with an efi truck.
i'm a 22re guy, but your v6 should also have a cold start valve on it, which is what puts additional gas into the motor when it's cold... make sure that the harness is plugged into it.

Check your engine coolant temp senor and AFM too.
After my engine was replaced it ran just like you said, hard to start, erratic idle. Engine ran perfectly fine in the other vehicle before the swap. Turned out the connector on the ECT sensor broke completely off. Probably when they were putting the engine in due to its location in the back of the engine.
That fixed the cold start issue then i had an erratic idle/power loss/hesitation issue. Turned out to be my air flow meter. Took the siliconed black cap off and I dont know how but there was oil in there all over the circuit board and connections. I used electronic cleaner to clean the AFM and had to fix some thin alum. connections under some pins in there and now it runs perfect.
