Second Opinion?

Not really a 'n00b' but I would like to get some opinions on my problem:
I have a '91 pickup with 450k on it. the engine was "supposedly" replaced about 20k ago but that was before it was given to me so I can't vouch for that.
It runs well, has all new ignition as of about 2k ago, I also pulled the fuel rail and injectors and went through that whole system. It had not given me any problems to this point, but I was driving home from work a few days ago and I noticed a loud noise. I thought at first it was either the car next to me or something stuck under my truck. I let of the accelerator and the noise almost went away. as soon as I put my foot back into it the noise came back. so I pulled to the side of the road. as I pulled off the engine was running a little rough and the oil light began flashing. I turned off the truck and went to put some oil in thinking I was hearing the valves and had somehow ran low on oil.... I put in 1 quart and then checked and it showed at 3/4 up the hash marks... so not low after all. I went back and started it and it idled just fine but as soon as I revved the engine the sound came back. this time the noise was much more pronounced and at a much slower tempo. there was a definite 'knocking' sound coming from the back of the engine (22RE). This was not a clatter, nor was it a rattle or anything that would lead me to think valves or lifters or timing chain. this sounded like the dreaded rod knock! I drove home very slowly and carefully and I could keep it from making the noise as long as the engine was at a low RPM.
I am thinking 3 things,
1. thrown rod (I don't really think so though because that would be more constant right?)
2. Wrist pin
3. Rod bearing
I am already prepping to pull and rebuild the motor but I wanted to get some technical opinions before I do.
What say you?

how's this coming along for you? did you ever find out what it is? i'm going though a similar issue right now ... i posted some video of my 22re running
