'91 SR5 Idle Issues

'91 SR5 Idle Issues
Hello,
I have a '91 P/U SR5, with a 3VZE and 5 speed. I burned a valve a couple years ago, and rebuilt the engine (slowly) over a year and a half. I was talked into having my cams reground instead of replacing them by the guy that rebuilt my heads. I also put headers on it.
The problem i'm having is idling. The truck starts easily any time I try, but when its' warm it will idle down and die. When it dies, I can restart it without problems, but it's still pretty annoying. Other than the idle, it runs great, and has good power from takeoff through 5500 RPM's.
Also, my CEL comes on, occasionally, when I'm on the freeway ( 60-70mph, 2400-3000 rpm's )
And ideas/suggestions?
