Missing Bolt on Coolant Pipe

Howdy everyone.
Was checking over my engine and noticed what looked like a small oil leak. I looked closer and noticed a bolt missing from the coolant pipe that connects to the back of the timing cover and runs alongside the block next to the headers. I called up my local toyota parts department and have the bolt on order.
Has this ever happened to anyone else?? Thankfully there is no coolant pouring out, but I am glad I caught it. Who knows how long it could have been missing.
The missing bolt hole right to the left of the bolt head

Most likely it was removed and never reinstalled .

Either forgotten or it was just to difficult at the time

The bolt got dropped and they just gave up
