'92 Pickup Milkshake - Headgasket or Timing Cover?

'92 Pickup Milkshake - Headgasket or Timing Cover?

Hey everyone - new here I have a '92 2wd pickup, 22re, with about 220,000 miles on it, and recently noticed milkshake on the oil cap and some evidence of oil in the radiator overflow (dark bubbly marks on the walls). Trying to figure out if it's due to a compromised headgasket or the timing chain wearing through the water jacket. Coolant was a little low and oil was a little high. Performed a compression test and these were the numbers:

1. 2. 3. 4.
D. 195. 180. 183. 182
W. 210. 200. 212. 200

To my knowledge it's never overheated. I've never seen the temp gauge climb past half way. It generally sits at about 1/3.
After noticing the milkshake I left it parked for about a week. When I turned it on to move it to a different spot in the driveway it made a loud knock / rattle for a brief bit then quieted down.
When we fired it up a few days later for the compression test, there was no rattle, and probing with a stethoscope it sounded pretty clean. Drove it around the neighborhood and everything felt good, but very obvious milkshake on the oil cap. Also lots of oil leaking/crusting around the valve cover everywhere, but that's been like that for a long time.
Any ideas what any of this means or what my next diagnostic step should be?
