Does your speedometer work?
Since your 1988 4Runner has a cable driven speedometer, there is a Vehicle Speed Sensor (VSS) built into the gauge cluster that gives the Engine Computer the Vehicle's speed to help it work it's magic.
According to the 22R-E Diagnostic Code chart on 4Crawler's website, code 42 is described as: VEHICLE SPEED SENSOR SIGNAL CIRCUIT--NO SPD. SIGNAL FOR SEVERAL SECONDS WHILE VEHICLE IS OPERATED UNDER HEAVY LOAD (TPS/MAP/AIR FLOW INPUTS).
This could be due to a bad wire, a poor connection, a bad speed sensor in the gauge cluster, or in rare cases, the ECU.
I don't have a 1988 Factory Repair Manual, , so I am not sure what the procedure is for checking the speed sensor, but a 1993 Factory Repair Manual says this:
2. INSPECT SPEED SENSOR
Check that there is continuity between terminals A and B
four times par each revolution of the speedometer shaft.
If operation is not as specified, replace the speedometer.
Maybe someone with a 1988 FRM can post up the procedure with the correct terminal locations for your 4Runner.
