Gents,
just did a Egr blockoff plate across the board and redid the vacuum lines as well. The truck starts just fine and idles higher and when it gets warmer it begins to surge and keeps going !

(1) Gasket vacuum leak at throttle body, could still be a vacuum line somewhere.
(2) Idle adjustment o-ring or idle just needs adjusting
(3) TPS
(4)Coolant bubble in system.
(5) AFM
I don't know what other work was done, so these are just some basic suggestions.
Since you deleted the EGR and whatever else, have you checked for any codes?

Still have a dashpot on there? Mine likes to stick when warm and I haven't figured out how to fix it yet, so I just have it set to not depress at all at closed throttle.
Also, the throttle cable can stick and seems to do so more once everything's warmed up.
