Toyota pickups date back to the 70's, so the 1st gen 4runner is NOT the same as the 1st gen truck.....the 84-88 truck is the 4th gen?
SR5 did not make the body larger...where did you hear that nonsense? All 4x4 trucks and 4Runners used the same body panels.
The 3.0 wasn't introduced until 1988, and an SR5 did not have to have it. There are many 4cyl SR5's running around. I had one myself.
As for SR5 options, they ranged and changed throughout the years. There is a table around here somewhere that shows what all they were. Can't seem to find it right this moment. 79-83 SR5's typcially had carpet, wing windows, woodgrain, nice stripe package, upgrade seats (ie bucket instead of bench) SR5 gauges, and a host of other things.
However, early toyotas could be ordered almost any way you wanted them. Nowadays, there are only 3-4 ways to "customize" your Toyota.
To anaswer the OP question, there are any number of things that could make it an SR5, but most common was upgrade gauges, seats, and interior. Fully optioned trucks got power windows, mirrors, Sunroof, power antenna, Altimeter/RO gauge, padded rollbar, and a few other nice things. Not all SR5's were fully loaded, and not all were V6.

in the 1970s, 5-speed transmissions began to appear in low priced mass market automobiles and even compact pickup trucks, pioneered by Toyota (who advertised the fact by giving each model the suffix SR5 as it acquired the fifth speed).

"SR5 stands for Sport Ralley 5 Speed. This is simply an upgrade trim package that was available similar to a Ford Lariat or a Chevy Silverado. Earlier model trucks had bucket seats, additional gauges/tach, stripes, carpet floors, and cloth interior. Later model trucks, although still SR5s, may have come with automatic transmissions. SR5 models do not necessarily come with air, tilt, chrome trim, power steering, or sun roof. Trucks with the SR5 package can usually be identified by finding an 'S' in the VIN code."

You never mentioned 4x4. Its true toyotas were available in 5-speeds, 2wd, quite a bit. However, 4 wheel drive models only had a 4-speed option, until the 22r came about (1981).

The Hilux pickup was first offered in a factory 4wheel drive configuration in 1979. This pickup featured a 20R 2.2L 4 cylinder motor backed by a L43 4-speed transmission and a RF1A gear driven 21 spline transfer case with the shifter on the top of the transfer case.
1981 saw the first major changes in the Hilux model. A 2.4L 22R motor replaced the previous 20R and could be found connected to a L45 4-speed or a L50 5-speed.

the SR5 meant sport rally 5-speed. I bought a 1981 new. It had no extra instrumentation, no stripes, no carpet. It was a longbed with semi bucket seats. I still have it. I am in the process of restoring it. I also have a 1982. Both have the 22R engine. I am trading engines. the 1981 has abut 400,000 miles and is losing power. I am surprised it didn't smoke very much.
