The warm slow starting issues could be an IAC valve.

What's an IAC valve, and where might it be.
Is there a way to check it?

If it were me, I would take off the throttle body and the IAC valve is attached to it. I would then take off the IAC valve and clean everything out. You will spill a little coolant because it runs through the IAC. That is a surefire way to tell where it is. I have never taken it off the 5VZ-FE engine. I have taken it off the 3MZ-FE (3.0 L V6) Camry Engine and the Honda 3.0L V6 VTEC engine. In about 1 Hour I had the whole thing out, stripped down, cleaned really good and put back together in the car. The 3MZ-FE is kinda similar to the 5VZ-FE. The IAC (Idle Air Control) valve is located underneath (on the 3MZ-FE) of the throttle body. My symptoms were: it would start when warmed up and then just die, or have a hard time starting when the engine was warmed up. The IAC valve was sticking. I used about 2 cans of Carb and Choke cleaner and cleaned it out really good, and you should notice black soot looking stuff (carbon buildup) on the butterfly and all through the little nooks and crannies of the throttle body. After I cleaned it out really good, I had no more problems. There is really no way to actually check it, you just have to open it up. When you do, if you run into rubber gaskets just put a little bit of oil on them to seal them back up when you put it back together. But a bad IAC shouldn't throw a CEL (check engine light).

Also IMHO, even if you open the IAC and it is not sticking, you should notice a difference in how well it runs if you clean it really well.
In fact, I may just clean mine out really good tomorrow since I am really not doing anything.
