HG Repl. 3VZE Almost done - LH Ground plug - HELP!!!

HG Repl. 3VZE Almost done - LH Ground plug - HELP!!!
Hey all,
About ready to fire up the Runner after HG replacement. I CANNOT figure out where the LH ground wire hooks to. The one end with the plug goes on the fender well but dows the other end connect to the head above #4 plug?
One more question - After filling the radiator, should I leave the cap off for the air bubble to escape?
I appreciate all the help and advice. Been a GREAT learning experience.
