Angry acceleration problem please help

Ok any help would be great here is what is going on when I start up my runner it idles at 2000 rpm and then when i put it into drive (I have a auto) it drops to 1000rpm but thats not the real problem the problem is if I do not let it warm up for about 10 min when i push the gas pedal my truck almost stalls and then revs up and starts to go sometimes even dies all together this is really frustrating when your trying to go through a light and it does this because the light usually changes by the time I get through the intersection. Please Please Please guys an help would be great. I have changed out plugs and wires and rotor and cap I also have done sea foam but nothing do you think it could be vacuum hoses? oh and in case you were wondering it does ok after the warm up please if you have anything that can help please let me know.

This is where I'd start. Just be careful with those plastic connectors, eh.
