Starting issue - ever hear of this?

Hi,

My '03 Tacoma base model 2.4l has an intermittent problem. Occasionally when the truck is warm and I make a short stop (7-11 to get a coffee, etc.), I'll come out & start it up but it won't idle. It starts up & promptly dies, and will continue to do this if I repeat normal starting. If I start it up and rev it fairly high, 2500-3000 I'm guessing, it'll idle fine, run great, and I'm on my way.
Does this sound familiar to any of you's? Any thoughts on what might cause this?

leaking fuel injector maybe? Coolant getting into the cyl somehow? Is there any smoke out the tailpipe while this is goin on? Black smoke would lead me towards excess fuel in cyl (leaking injector, etc..) White smoke would be coolant (hg, intake gasket, etc..) and bluish smoke would be oil (rings, etc...)
