I have gotten mine from Partsgeek and Mossy Toyota (I forget the actual name of their site) and RockAuto, all with good luck, including this particular job on my 92, detailed elsewhere on this site

Heavy white smoke is head gasket till proven otherwise. If smells sweet, there is the answer for sure. A little bit of milky color on the oil cap would go along with the diagnosis.

No idea. I am thinking that either just spontaneously crapped out, or maybe during the A/C compressor replacement a bolt got loose (am missing one from a line bracket) and went into the fan, etc....
