Low power and slight shudder from start - 1995 T100

I just bought a 1995 T100 with 247,000. It’s my first one! It has a slow shudder and poor acceleration from start. It seems like the idle might be a bit low too. Any suggestions to the cause? It’s seems to run really well once I’m over 10mph and even at high speed it’s great. If it is warmed up, the problem seems to be minimized too. Thanks.

2WD or 4WD?
Manual or Auto?
4 cyl 2.7. 3.0 or 3.4 v6 (pretty sure all 3 were available)

The more info you give the easier it is for people to help...
