90 22re not running good, cannot figure it out

Alright heres the deal.

I have a 90 toyota dlx 22re automatic. I have never had any problems with it and the original ac still works. the truck has always had its maintance and everythign works great. in its time i have only replaced a battery, altenator and water pump. that seems pretty good to me for being a 90.

recently i noticed i was getting gas a lot more often and that the idle was higher then usual. sometimes it would idle higher as if i was turning on the ac or somehting but without actually turning it on. then after a little time and not being able to figure out the problem things beagna to get worse.

now at this point i am totally lost on what is going on.

when i start the truck cold, like in the morning after sitting and it runs terrible. it seems like it is running on 2 or 3 cyliners. or almost like it is consistanly missing, or there is a lack of air. its realy weird. you can smell through the exhaust a weird gaseous smell. after some time it will clear out and run fine except for the higher then usual idle. now this issues is different everytime. sometimes it will start up and be fine and other times it will clear up in 30 seconds and other times it will take 10 minutes. then if i am sitting at a light it will miss for a second or 2. it is totally intermitant and is different everytime. but every day it does seem to get worse.

I need some help trying to figure it out. maybe someone on here has seen this before. i cannot totally explain the issue but that is my best description.

spark plugs or wires maybe. maybe vacuum leak. maybe faulty idle air valve under the throttle body.
personally, I'd lean towards either the vacuum leak or IAV being the problem.

I have changed plugs wires, cap, doesnt seem like there is any vaccum leak, i check the best i could and no audible signe of a vaccum leak, it has never thrown a check engine light either, even when it seems to be running on 3 cylinders.

I have tried this in the past when the idel issue first came about. i could not find any leak at all.

the truck is not running very good at all. it runs what seems to be like 3 cylinders but i wont stall. this morning it tooka while before it started it seemed like it was running all 4 cylinders. the truck never looses any power which confuses me even more. it sounds like its a diesel or some thing. its weird i dont really know exactly how to explain it.

is it possible to pull codes even if the check engine light is not on. i dont think any of this has ever caused it to throw a code.
