3RZ-FE valve troubles

Engine
2002 Tacoma, 3RZ-FE (2.7L), California emissions, 150K

Symptoms:
Persistent misfire in cylinders 1 and 3 and multi; once daily occurrence of either one of or a combination of P0301, P0303, P0300, when the engine is at operating temp. (for the past year)
Rough idle and running. (possibly for the last 2 years)
Sluggish performance. (possibly for the last 2 years)
Slight tapping/ticking (knocking when cold). (for the last 10 months)

Measurables:
Compression; spec pressure 178psi or more, minimum pressure 127psi:

Readings:
Cyl 1 – 75psi
Cyl 2 – 175psi
Cyl 3 – 55psi
Cyl 4 – 190psi

Valve clearance; spec Exhaust – 0.25-0.35 mm(0.010-0.014 in); spec Intake – 0.15-0.25 mm(0.006-0.010 in):

Readings in order from front to back:
Exhaust:
Cyl 1: less than 0.038mm(0.0015in), 0.127mm(0.005in)
Cyl 2: 0.102mm(0.004in), 0.178mm(0.007in)
Cyl 3: 0.064mm(0.0025in), 0.127mm(0.005in)
Cyl 4: 0.229mm(0.009in), 0.229mm(0.009in)
Intake:
Cyl 1: 0.229mm(0.009in), 0.229mm(0.009in)
Cyl 2: 0.203mm(0.008in), 0.229mm(0.009in)
Cyl 3: 0.203mm(0.008in), 0.203mm(0.008in)
Cyl 4: 0.152mm(0.006in), 0.152mm(0.006in)

The intake valves seem to require a slight adjustment but the Exhaust valves are way too tight. I have an opinion what this is indicative of, but want yours before I tear the head off.
