You're kinda going against the general consensus about what tires and wheels work the best. 33x12.50s are too wide. You have to do a lot of cutting to get them to not rub. And you'd be making it much worse with 3.75" BS. If you don't want to hear them rub on the firewall on every bump and turn, don't go with anything wider than 11" and stay within 4-4.25" BS. That's why the vast majority of lifted 3rd gens run 285/75/16s (33x11s). The problem with 15" wheels is most of the 33s are 12.50" wide. 33x10.50s will work but there aren't many choices in that size. 16" wheels are way more popular on our years mainly because the metric-sized tires fit better.
5" backspacing will not work unless you run wheel spacers and effectively change the BS to ~4". Wheel spacers are a lot cheaper than new upper control arms though. Another reason why I wouldn't recommend 5" BS is because your tires will be sucked up in your wheel wells and your stance will be skinny. That means your center of gravity will be higher.
My advice...
Get 285/75/16s with wheels that have 4" BS. OR, 255/85/16s (33.5x10) and wheels with 4-4.5" BS.
