Wheel and tranny questions

I tried a search and didn't turn up many results so I'm going to beat a dead horse

I just picked up a 92 4Runner SR5, 4wd, V6, auto for $500. It has a weird quirk in the tranny. Occasionally it gets stuck at 40 mph and won't go any higher. It has no problems reaching 40 but just stays there. Any ideas?

Also, I want to put some wheels on it because I'm a bit of a wheel whore and also because it has 3 different wheels on it. I've figured out the lug pattern but am unsure of the lugs, if they are conical or not. What are the differences in lugs? I am really interested in a set of wheels like the BlackRock Street Lock series wheels but they don't have to be BlackRock specific. I'd like to get something with an offset as close to stock as possible, 4.25-4.75 if I'm not mistaken? I think I'd like to run either a 7 or 8" wide rim. Any recommendations?

As far as your transmission problem, do your revs get really high at 40, or does it seem like it's in the right gear, just not going any faster?

The revs don't seem to get any higher when the problem does exist.
