Engine problem question

Hey guys
I've got a little of a question for you guys, my buddy (who is a machanic) has an 1989 (I believe) pickup with a 22re in it,(he is working on it for another buddy of mine) the engine just started acting up. When it is idling it runs really rough, like it is miss firing and it will run like this in all the forward gears but when you put it in reverse and "go" the engine runs just fine like it should.
He is really stumped on whats wrong with it, so I thought I would get on here and ask you guys if theres anything you can think of that could be going on with it.

engine twisting maybe messing with grounds or something?
in forward gears engine will twist one way in the engine area and if reverse it will twist the other is all i can think of

Thanks an intriguing problem. NEVER heard this one before! I can't think of a single thing except that maybe a wiring issue that involves the reverse switch. Has anything been re-wired or added to the electrical system? I've seen some bizarre stuff after folks have installed radios, burglar alarms, & trailer light kits....

I don't know what all has been done with the trucks wiring but I will let him know to look at maybe a ground wire or something. Anymore ideas? Oh and don't suggest taking it to a toyota dealership he took it to one and they had it for 2 weeks and couldnt figure it out......

i would unwrap the harness from the injectors to the firewall and inspect the wires, i had a 88 that had 1 corroded injector ground wire and it took me forever to find it.if you bend the wire and its crunchy inside...well you get the idea.
