95 4runner 3vze Belt Alignment Problems

95 4runner 3vze Belt Alignment Problems
So i just replaced the timing belt, water pump, fan bracket for my girlfriends truck. That all went swimmingly.
Reason I replaced was as we were driving down the road the whole fan assembly blew out and forward, truck died out. So I had to replace that bracket, look inside, belt is shredded.
So after the replacement, which went great, truck was running fine for 3 days.
Today the alternator belt pops off because the power steering belt is pushing it out. Now it looks as if the fan and the pulleys attached are pulling themselves forward towards the radiator again. There is about a half inch of room where it seems like it should be more aligned.
Basically I don't know what to do, or why it is happening. I am at a loss.
Alternator belt is falling off due to power steering belt rubbing against it. It looks like the power steering belt is rubbing because it is being pulled out at a weird angle.
Will try to post pictures, we are in the middle of nowhere on bad satellite internet.

Tried taking some car porn pics, hard as a self made producer.
So yeah, I don't know why but it seems like the fan is pulling out. The picture shows the deflection from the PSP. Tried also taking a top down but yeah. Will attempt clearer pictures soon.

Better Shots
Got some better pictures now, you see if i was to tension the alternator again it would just pop the belt right off once i start it up. No clue why it is happening.
The bracket that holds the fan assembly is new, bought for 278$ at napa. The old one exploded out. I also replaced the timing belt and water pump. Again the truck ran fine the last 3 days, no squealing no problems. And to the best of my knowledge the belts lined up initially.

Found the issue
So why would the fan assembly bracket be getting pulled out. This would be the 2nd time it is happening. The power steering pump and the alternator are aligned perfectly with the crank, so I don't know how it could be getting pulled out.
Any tips or thoughts. I am at a loss.

The bracket matched up perfectly to the one that was one the truck, and there is really no other way to install it. I had it flush to the engine and torqued to spec. I appreciate the responses man, I really do.
