Timing the 5VZ-FE

If I have the crank pulley at the zero mark on the harmonic balancer and the two cam pulleys on the marks at 12 o'clock is there any way that I could be 180 degrees out of time? I have searched and searched on the site and I can't find any reason why my belt would not be correct but it is running poorly. It is skipping and has no power but if you rev it past 3000 rpms it smooths out and sounds good?

I did read an article where a crank pulley was missing a tooth for the crank position sensor and I am planning on checking that tomorrow. I am at a loss right now after this engine build.

Does it really matter if you do the 2 dots or 1 dots on each side? In reality the dots are 180 degrees apart on each cam shaft. So if you timed the exhaust cams on the 1 dots on each side when you spin the intake cam 180 degrees you would be on correct? If so, then as long as you line up the cam gears on the timing marks the heads should be right? I am a little at a loss as to why I appear to be out of time but every thing lines up?
