How do you and which pins to check codes?

Can someone give me the basic run down on the procedure for pulling the codes from my 92 truck? Start at the begining. As to which pins to jumper and so on, and where I can find out what the code's mean after I pull it.

My CEL is posesed. It will come on and go off some times and other times it will come on and stay on. I pulled the EFI fuse and left it out while I was at work for the day to try and reset it to see if it was a fluke, but the CEL came right back as soon as I started it. It seems to be possibly emmisions related before I pull the codes. The times that it comes and goes, it does not come on when idleing or maintaining RPMS, but when I accell or shift it will come on and after I reach a steady RPM it will turn off.

Thanks and I will let you know what is up after I get someone to tell me how to do it.
