Front Shock Top Mount

Front Shock Top Mount
Well she's getting up there in miles and the little problems have started. This round includes and wild little chirping noise coming from the passenger side front wheel well. I think I've pinpointed it to the strut mount because the top bolt seems higher than it should be because the big flat washer looking thing under it (between the bolt and the top arm) wiggles around pretty freely and when the suspension compresses, it rubs a little.
I tried tightening the bolt but I just twist the entire shock tube and when I try and clamp the square head that sticks out of the top, I just can't seem to get enough torque to twist the bolt by itself...
Any ideas?

Im in the proscess of putting a leveling kit on and when i went to remove the strut all I had to do was put an adjustable wrench on the square head and had a buddy hold that while I loosed the nut with a wrench. it took some torquing to take it off but we got er fairly easy.
