Quick Tranny Q

My Tranny seems to be shifting rough, especially if I try to shift somewhat quickly (i.e. the rpms are still high). If I go slow and let the RPMs slow, it shifts smooth. Could this simply be a need for A Tranny Flush/New Fluid? I heard about changing that rubber ring that sits between the shift lever and the tranny as well. What do you guys think?

FYI: I bought the truck about 35000Kms ago (Has 295,000 Kms), and I have no idea when the last time the tranny fluid was changed...
