Exclamation Fuel puddled in Throttle body & takes a couple tries to start from a cold start.

My little brother has a 1994 Toyota pickup 3.0 4x4. It has in recent weeks begun to have issues from a cold start up it takes 2-3 tries for it to start and I can smell strong gas. I took it by a local mechanic and he said he thinks a injector was leaking. So I went home and went to take the intake and all off to check them and replace and I noticed there was no gas around the ones he thought might be leaking under the throttle body on the driver side. I took the intake hose off and there was fuel inside the intake hose and a puddle of it inside the throttle body. I turned the key on off couple times watching the injectors and then the inside of the throttle body and no fuel sprayed out. Then I dried all the fuel up in the throttle body and started the truck and let it up and still no fuel inside the throttle or any leaks. I let it sit for an hour to cool off and then I went back out and after sitting for an hour it has puddled fuel up in the throttle body. Any clue what is causing this problem with the startup and fuel puddle in throttle body?

My little brother is driving one of my vehicles at moment until I can fix his because I feel this might be unsafe.

Forgot to add when he first started having this issue I thought maybe the fuel filter was clogged so I have replaced it.

Has your brother had the truck apart recently? It's easy to mix up a couple lines and connect a fuel return line to a vacuum line. Don't ask me how i know. If he hasn't had the truck apart then the only injector that would pool fuel in the intake plenum would be the cold start injector. Located on the passenger side of the intake plenum. I would start troubleshooting there. Do you have an FSM? If not there is an online version stickied somewhere on this site. Good luck.
