93 4Runner not shifting into 2nd gear
I have a 93 4Runner that was working perfectly until engine coolant leaked into the transmission. Had a new transmission put in and everything was wonderful for maybe 2 months, then it started revving higher and higher to go into 2nd gear. Finally, it got to the point where it would miss 2nd gear completely and rev to over 3000rpm then shift into 2nd and third at the same time.
I took it back in, and they took it apart and replaced the clutch. That didn't fix it, so they put in a new speed sensor and kickdown cable. They said it was good.
It wasn't. It is still doing the same thing, only now it revs to over 4000 before it shifts.
I am taking it back in today. What else could it be?

Throttle Position Sensor.
The "kick down" cable doesn't have anything to do with your problem.
Of course, I'm not there, and your shop is. I can only imagine how annoying it is for a customer to come in and say "but some guy on a forum says ...."
More importantly, YOU can check the TPS yourself.

I just noticed that it gets worse as the longer I drive it. I started it up to drive it to the shop, and it worked fine. After driving it for about 20 minutes, it started doing it slightly. When I got to the shop and took the mechanic for a test drive it was REALLY doing it.
That explains why they gave it back to me claiming it was good, as they only took it for a 5-10 minute test drive.
Any ideas what it might be based on these symptoms?

At lower temperature, the ECU prevents the transmission from shifting into overdrive or lockup. http://www.autoshop101.com/forms/AT10.pdf
That's not your symptom, but given that you have temperature dependence I would check that sensor (the bad news: there are four temperature sensors in the water bypass at the aft end of the block; I can't tell you which is which).

Both the ECT and overdrive work as they should. I push the button, and the dash lights come on and it does what it should.
I did notice, though, that it doesn't even shift into 2nd if I do it manually,

Yes, I have driven it for years with no problems until the coolant line broke. That was back in October. Once that was fixed, it drove fine until January/February when it started doing this new thing.
