91 4Runner - Random click / clacking sounds at low speed

I've got a '91 4runner. 230,000 miles. No lift but its on 31's with aluminum rims.

I've got a strange problem that I can't seem to pinpoint the source.

At low speeds it makes a clacking sound and it sounds like its coming from the rear wheel(s). It sounds like a piece of steel hitting a rock, just a solid clack and it only seems to happen at low speeds. Its very inconsistent and doesn't always happen so its hard to replicate. This noise has been around since I got the truck (4 months)

I've just replaced the front CV shafts and shocks. Also the ball joints feel tight. I can't feel anything moving in the front or rear suspension and nothing looks worn. I don't hear any noises at high speeds either, only in 1st or 2nd gear when taking off or slowing down. It doesn't make any difference on hills or flat. Also turning left or right doesn't change anything.

My thought is it may be drum brake related? I have no idea.
Anyone run into this before?

Have you checked the u-joints? Is the rear diff filled with the proper lubricant?

Yes, could be something in the brake drums too.

Put the rear axle on stands and properly blocked..then put gear in neutral/ release parking brake and then manually spin tires to see if you can isolate the sound...random is hard but it might be there all the time but you can't hear it because of road noise...my first guess would be u-joints, brakes...and then diffs.

Yes I did check the U joints. I replaced 3 of 5 and the lest 2 were still solid. Also I replaced the front and rear diff fluid when I got the truck.

I will try putting it up on the stands and try to isolate it. Thanks.
